Intake and Referral Officer, Adult Services

Westhaven is seeking an Intake and Referral Officer to ensure a responsive and proactive intake experience for prospective participants and their families.

About You

As an Intake and Referral Officer, you will you provide advice and support to potential clients and their families to ensure a positive first point of contact with the organisation and deliver an exceptional onboarding experience. You will:

  • Provide positive first contact by efficiently addressing email & phone enquiries
  • Collaborate with participants and families to understand their unique needs
  • Partner with service delivery to match services to specific care needs
  • Uphold NDIS principles and relevant legislation in all processes
  • Generate insightful reports on client data, trends, and positive outcomes
  • Proactively engage with funding bodies to facilitate resource access
  • Build strong relationships with internal and external stakeholders for seamless collaboration

 

You will have:

  • Strong communication skills to forge and maintain positive relationships, contributing to a collaborative environment
  • Confidence to uphold service user privacy and confidentiality
  • Adaptable, exceptional organisational and problem-solving skills to thrive in a dynamic, high-volume setting
  • Keen attention to detail and take proactive initiative
  • Ability to work working autonomously to drive results
  • Proven experience in managing competing priorities to consistently deliver high-quality outcomes within agreed timelines
  • Sound understanding of person-centred thinking, the principles of the NDIS and the National Disability Service Standard
  • Certificate IV in Disability, Mental Health or Community Services or the equivalent demonstrated knowledge and/or relevant working experience highly desired

Compliance requirements:

  • Current NSW Drivers licence
  • Obtain and maintain a current paid Working with Children Check (WWCC)
  • Obtain and maintain a valid National Disability Insurance Scheme Workers Check (NDISWC)
  • Willingness to undertake a pre-employment police check
